Case Manager
Job Summary
The Utah AIDS Foundation is seeking a full time staff member to provide case management services to HIV-infected clients to reduce the barriers to obtaining/taking medication and accessing medical care.
- Conduct intake interviews with potential clients to assess needs and development of an appropriate service plan.
- Provide on-going individual case management and referral to supportive services to HIV-infected clients.
- Facilitate support group for people living with HIV, specifically gay and bisexual men.
Knowledge, skills and abilities
- Adaptable, changeable, and willing to provide a variety of services daily;
- Written and verbal communication skills;
- Ability to communicate effectively;
- Ability to be self-motivated and self-directed.
- Computer literate and proficient with Microsoft Office products, especially Word and Excel.
- Bilingual preferred (English-Spanish)
Education, experience and licensing and certification
- Bachelor’s degree in human services related field required; Master’s degree preferred.
- Social Service Worker (SSW) eligibility or licensure required.
- Ability to relate positively and sensitively to persons diverse in lifestyle, age, ethnicity, beliefs, socio-economic level and sexual orientation.
- Must have reliable transportation for travel to client’s appointments and other statewide travel.
Compensation
- $28,000 to $32,000 per year DOE
