Please use the form below to register for the Utah AIDS Foundation golf tournament. You can register for up to four teammates. You can also register by calling Nathan at 487-2323 or you can download a registration form in a Word document by CLICKING HERE

Cost per player is $60.00.

If you are paying for someone who is not on your team, please do so in a seperate transaction. After you have completed each section, select "Add To Cart", and then select "Continue Shopping" in order to move to the next section.

If there are players on your team that you are NOT paying for, please list their names in the "Additional Team Members" field, so that we can match you with your appropriate team (this field is located on the Paypal page during the checkout process). If you would like us to match you with a group of players, please leave that field blank.
