Case Management

The Utah AIDS Foundation offers case management services, free of charge, to anyone living with HIV/ AIDS, including children, adolescents, and their families. 

If you, or members of your family, are living with HIV disease and need help planning, coordinating, or accessing any of the services available to you throughout the state of Utah, a UAF case manager can help.  Understanding all the different programs and types of assistance can be confusing and difficult.  Our case managers are here to help you sort through it all and get the services that will help you manage an HIV infection.

If you are HIV+, on Medicaid, Medicare or Medicaid/Medicare and are confused about Medicare Part D prescription drug plans, our case manager can answer your questions.

The primary goal of UAF case management services is to assist people living with HIV disease in accessing and maintaining regular medical care.  This means a UAF case manager can help you with transportation, housing, mental health services, substance use treatment, support groups, food and meal services, benefits advocacy, financial assistance, and much more.  In short, case management is here to help you access a variety of community assistance programs.
